Nursing Outreach and Home Medication Delivery.

We know that many patients have difficulty attending a pharmacy frequently to take their medications. Some have mobility limitations, parenting and family responsibilities, challenging work schedules, supportive housing requirements, or some days may be filled with other important engagements to support their recovery. We have a professional partnership with Nursing Outreach Teams who can assist with daily medication administration where YOU are if it is difficult to attend a pharmacy. You can talk to your Metro City doctor and the Care Coordination Team to see how our Nursing Outreach Team partners may help. Recovery can be hard. We do our best not to make it harder.
