Nursing Outreach and Home Medication Delivery.
We know that many patients have difficulty attending a pharmacy frequently to take their medications. Some have mobility limitations, parenting and family responsibilities, challenging work schedules, supportive housing requirements, or some days may be filled with other important engagements to support their recovery. We have a professional partnership with Nursing Outreach Teams who can assist with daily medication administration where YOU are if it is difficult to attend a pharmacy. You can talk to your Metro City doctor and the Care Coordination Team to see how our Nursing Outreach Team partners may help. Recovery can be hard. We do our best not to make it harder.
Primary and Specialty Care Partnerships.
For some patients, getting stable on opioid agonist treatment can provide the first chance in a long time to deal with other medical problems and mental health challenges. It can also be the first chance in a long time to take preventive steps for your health like screening for chronic illnesses. Metro City has developed partnerships over the past 20 years so that we can help connect you to care providers, many of whom can see you right at a Metro City location where you feel comfortable.
